1. This offer valid only with the purchase, lease or installation of Motorola Canopy Subscriber Module with activated service between 4/1/2007 and 6/30/2007 2. To receive your rebate you must mail the original Mail-In Rebate Form with the required supporting documents to the address listed above, rebate requests must be postmarked on or before 1/15/2007 and received by 1/30/2007.  3. Limit one rebate per home.  4. Offer good only in North America (U.S. and Canada) and void where prohibited, taxed or restricted by law.  5. Motorola Canopy Subscriber Module cannot be returned once this rebate form has been submitted.  6. Not responsible for lost, late, mutilated, misdirected or postage due mail.  Illegible or incomplete rebate forms will be considered invalid and ineligible for rebate.  7.
